One issue parent would commonly observe as their children learn to walk and run is the inward twisting of the feet. This phenomenon is commonly referred to as pigeon toes or in-toeing.

Adults can also experience in-toeing. When this is realized later in life it can be harder to correct fully. The aim is to correct where possible and to reduce and associated symptoms such as pain, thick skin build up or wounds.

What is In-Toeing?

In-toeing occurs when feet turn inward instead of pointing forward while walking or running. This condition is colloquially referred to as "pigeon-toed."

While parents often first observe in-toeing when a baby takes its first steps, it can manifest in children of various ages for different reasons. Adults also may experience this condition, it may have gone unnoticed for many years until it becomes more noticeable, or pain occurs.  In-toeing can stem from three primary factors:

​

  • Metatarsus adductus (inward turning of the foot)

  • Tibial torsion (inward rotation of the shinbone)

  • Femoral anteversion (inward rotation of the thigh bone)

​

In most cases, in-toeing tends to resolve on its own without the need for braces, surgery, or other specialized care, especially in children under the age of 8.

​

In-toeing itself does not lead to pain or arthritis. However, if a child's in-toeing is accompanied by pain, swelling, or a limp, it is advisable to consult an orthopaedic specialist.

What are the Symptoms of In-toeing?

With in-toeing, individuals typically exhibit the following foot positions:

  • At rest

  • Standing

  • Running or walking

​

Due to their pigeon-toed gait, children with in-toeing are more prone to tripping and falling when moving quickly. While in-toeing does not directly result in arthritis, persistent in-toeing issues into adulthood may increase the likelihood of muscle strain/pain, stress fractures, hard skin buildup, pressure wounds and arthritis development.

Treatment Options for In-toeing?

  • Footwear changes

Many kinds of footwear do not offer any support or control for patients who in-toe. The heel can be slanted to increase the angle of the foot and ankle. Some cases may require a stiffer shoe or a higher-cut shoe.

  • Orthotics

These custom-made insoles can be used as a correction method to increase stability, decrease tripping and prevent pronation compensation. This helps by supporting the arch of their foot and preventing heel eversion.

  • Exercise prescription

In-toeing can be related to over or under-active muscles. A person may have hypermobility or lose ligaments that contribute to this condition. A well-rounded exercise program can help with alignment and pain and strain related to this condition.

  • Bracing

In cases of severe metatarsus adductus or when in-toeing doesn't naturally improve, your child's doctor may recommend foot and ankle or leg bracing to help realign the soft tissue and bones over time. it can be difficult to wear such devices and they may be hot or uncomfortable. Parents may find it difficult to encourage their child to wear leg bracing regularly. While it can be very helpful for some patients that are exhibiting signs of severe in-toeing it is not for everyone. Other more conservative methods should be considered before considering this treatment option.

  • Surgery (osteotomy)

For adolescents with severe tibia-related pigeon toes causing functional challenges, a paediatric orthopaedic surgeon might suggest surgery. While femoral anteversion surgery is not usually advised, it could be considered for children older than 11 exhibiting severe in-toeing symptoms. Before considering surgery all conservative non-surgical options should be exhausted.
